Please keep in mind a clan doesn't have to be blood related. It could be an organization of sorts practicing special secret ninjutsu, taijustu etc.

Clan Founder: Name of the founder?
Clan Name: Name of your clan?
Clan Weapon: Special weapon of the clan, for an example the the kaguya clan uses there bones.

Village: In which village is your clan located?

Physical Traits: A trait that is typical for the clan. For example all the Uchiha have black hair.

Bloodline: What is the name of your bloodline? For example: Sharingan.
Ability: What is the ability of this bloodline?

Clan History: Please give us a good clan history.

Clan Techniques: These techniques will be unlocked when you reach the required rank. You're allowed to have three Genin, three Chuunin, two Jounin and two S. That's 3 D-Ranks, 3 C-Ranks, 2 A-Ranks, and 2 S-Ranks. You may not exchange these in any way, though you may have less than the max amount stated.
